Why did they do it?

The authors were curious as to whether there was any real, measured benefit to continued “maintenance” or “wellness” visits beyond the initial phase of care in regards to CHRONIC low back pain.

It’s a great question that needed to be asked because patients come into our offices concerned about having to come see a chiropractor a million times, etc. Is there any real benefit?

How they did it.

  • Single blinded, placebo-controlled
  • 60 patients
  • All chronic
  • They had to have suffered from the pain for at least 6 months.
  • Randomized to receive either
  1. 12 adjustments of fake treatment over a 1-month period or
  2. 12 treatments of actual treatments over a 1-month period with no treatment in the following nine months.
  3. 12 treatments of actual treatments over a 1-month period followed by manipulation every 2 weeks for the next 9 months.

They measured pain and disability scores, generic health status, and back-specific patient satisfaction at 1 month, 4 month, 7 month, and 10 month intervals.

What They Found

Patients in the manipulation groups had significantly less pain and disability than did the first group when the 1 month period was up.

***Only the third group (the one with follow up maintenance treatment) showed more improvement in these scores at the 10 month evaluation. ***

This study indicates that maintenance spinal manipulation offers the best long-term benefit.
