Research: Manipulation/Mobilization for Neck & Back Pain

Unfortunately, there has not been enough research to show the true effectiveness of manipulation/mobilization in the case of neck pain. Until recently, it has been mostly anecdotal, meaning that we see people improve almost immediately most of the time but solid research showing this just hasn’t been performed.

Realizing the vacuum, many researchers are starting to pick up the pace on this and we are starting to see more and more research addressing this issue. It is starting to look better and better for Doctors of Chiropractic.

WHY THEY DID IT

The paper did not focus on cervical pain but rather it included cervical pain in a more broad paper. The authors wanted to proved a summary of what was already out there in regards to how effective manual treatment has been for musculoskeletal complaints as well as non-musculoskeletal complaints. 

HOW THEY DID IT

  • This study was a systematic review of randomized clinical trials
  • The strength and the quality of the evidence was based on a grading system.
  • 26 categories of conditions involved in a Randomized Clinical Trial were identified for manual therapy
  • 13 were identified for musculoskeletal complaints
  • 4 kinds of chronic headache
  • 9 non-musculoskeletal conditions were identified


WHAT THEY FOUND

Spinal manipulation/mobilization is effective in adults for

  • Acute low back pain
  • Subacute low back pain
  • Chronic low back pain
  • Migraine
  • Cervicogenic headache
  • Cervicogenic dizziness
  • Several extremity joint conditions
  • Acute neck pain (thoracic adjustment)
  • Subacute neck pain (thoracic adjustment)


There was inconclusive evidence on whether cervical manipulation/mobilization by itself is effective for acute/subacute neck pain. This is precisely why Doctors of Chiropractic commonly combine other treatment modalities for neck pain like massage, exercise, and physiotherapeutic modalities. This is also why there needs to be more research on cervical pain and manipulation.
