RESEARCH: Manual Therapy & Exercise in General Low Back Pain

Why They Did It.

The researchers wanted to review all evidence they could find on different types of manual therapy used on different patients throughout different stages in non-specific, general low back pain. That sounds like a heck of a good reason to me.

How They Did It

The researchers used Medline, PEDro, Cochrane-Register-of-Controlled-Trials, and EMBASE for their data mining. The time span for the studies they used ran from January 2000 to April 2013. Thirteen years is a pretty solid sampling. The review of all of this literature was done while following the Cochrane and PRISMA guidelines so it was formatted accordingly.

  • 360 studies evaluated
  • Two stages of low back pain were used (acute-subacute and chronic)
  • Manual therapy types were subcategorized: MT1 (manipulation) and MT2 (soft tissue techniques and mobilization)
  • Also, there was a MT3 which was MT1 and MT2 combined
  • MT could be combined in usage with exercise or usual medical care.

What Did They Find?

STRONG EVIDENCE – MT1 had strong evidence on it’s side compared to fake treatment. It was effective in the treatment of pain, function, and health improvement in the short-term (1-3 months).
MODERATE-STRONG EVIDENCE – MT1 and MT3 combined with usual medical treatment in comparison to usual medical treatment alone. MT1 and MT3 with medical treatment was MORE effective than medical treatment alone for pain, function, and over health in the short-term.
MODERATE EVIDENCE – MT3 with exercise or usual medical treatment compared to exercise and back-school. MT3 with exercise or usual medical treatment was superior for the quality-of-life int eh short and long-term.
